The Patient Forms page may at times be the most useful page on your web site.
Your patients will always prefer to fill out your practice’s forms in the convenience of their homes, when they have the time available. With the variety available through our Patient Forms program, you can place every type of form your practice uses on this page.
After we convert your forms to either PDFs or WebForms, we’ll place an icon for each form on the page, along with a link to download Adobe Reader® software (if applicable).
Copy should be added to the page instructing your patients to click on each form icon, fill out the form (either on-line, or by printing and filling them out by hand) and bring them to their first appointment. A reminder to current patients to fill out a new patient information form whenever their personal or insurance information changes is useful.
Adding patient forms to your web site helps in many ways. Using online completion forms eliminates the legibility problems that your staff encounters so frquently, and our online submittal forms allow your patients to send your forms in to the practice before they ever arrive.
